How to File a Complaint with the Texas Medical Board

Blue Agave Functional Medicine prioritizes your health and satisfaction. If you have concerns regarding medical services received via telemedicine, you have the right to file a complaint with the Texas Medical Board (TMB).

As required by the Texas Medical Board:

Complaints about physicians, as well as other licensees and registrants of the Texas Medical Board, including physician assistants and acupuncturists, may be reported for investigation at the following address:

Important Follow-Up Requirement

To ensure timely review of your results and appropriate medical care, a mandatory 30-minute follow-up appointment must be scheduled approximately 2 weeks after any New Patient visit.

If this follow-up appointment is not scheduled, review of results and medical recommendations may be delayed due to provider availability.

By proceeding with scheduling, you acknowledge that it is your responsibility to schedule this required follow-up appointment and understand that failure to do so may result in delayed follow-up and/or delay in care.
